Off-Campus Permission Form
I give my son/daughter permission to leave the CTech campus during lunch

ONLY to patronize stores and/or restaurants to purchase food items.

Students may go to stores and restaurants up to the following locations:
- Taco Shack to the north.
- Agat Oriental to the east.
Students will sign out at the Main Office before leaving campus and sign in upon their return.

Students must be on campus and ready to attend their next class upon their return.

Any violations of the Off-Campus Lunch Policy will result in the following:
- First Violation: Warning, report to the Principal
- Second Violation: Suspended Privileges for Two Weeks
- Third Violation: Suspended Privileges for One Semester
