I/We, the undersigned parent(s) or legal guardian(s), request enrollment of the above listed student in Holy Cross Preschool for the 2023–2024 school year. I/We agree to abide by the following conditions of enrollment:

1. I/We agree to pay the non-refundable deposit by March 30, 2023. If payment is not received on time, the place in the preschool will not be reserved.*

2. I/We agree to the details indicated in the 2023-2024 Enrollment Contract Addendum. The 2023-2024 Tuition & Fees Schedule (Preschool/Pre-Kindergarten)will be in effect through June 1, 2024 (Academic Program) or July 26, 2024 (Academic Year & Summer). We will be given at least 30-day notice of any schoolyear tuition schedule change that occurs after August 1, 2023.*

3. I/We realize that the school agrees to collect tuition as follows: For enrollment in the Academic Program, a 10% deposit is required by March 30, 2023; the remaining balance is payable in 10 monthly installments starting June 2023 and ending in April 2024. For enrollment in the Academic Year & Summer, a 10% depositis required by March 30, 2023; the remaining balance is payable in 11 monthly installments starting July 2023 and ending in May 2024.Tuition management and payment processing are managed by FACTS Management Company*

4. I/We agree that a late fee of $25 will be added monthly if complete tuition payment is not received on time. Should the bank dishonor my/our payment, and an additional $35 fee will be charged. Tuition and other fees that are 90 days past due are reasons for dismissal and/or refusal for acceptance of the child. Collection procedures for delinquent accounts will be handled by Credit Consulting Services, Inc.*

5. I/We agree to pay a late fee of $5.00 per minute, with a minimum of $20, should I/we arrive late to pick-up my child. Pick-up time will be no later than 4:30 p.m. No exceptions will be made to this policy*

6. I/We understand that the deposit is non-refundable. No credits are given for absences, holidays, vacations or unforeseen events that may arise. I/We understand that if I/We decide to withdraw my/our child/children during the school year, tuition for the current trimester will be paid in full. Refunds, if any will be processed providedthe school is informed, in writing, at least 30 days prior to the date the child/children is withdrawn. An early withdrawal fee may be charged to cover any upfront expenses.*

7. I/We agree to comply with the educational policies and regulations of the Diocese of Monterey and Holy Cross Preschool as set forth in the Holy Cross Preschool Family Handbook.*

8. I/We understand that the students may be terminated from the program for the following reasons: responsible party’s failure to meet financial obligations; consistent behavioral problems with the student; actions by family members of the student that negatively impact the school; and consistent lateness in picking up the student.*

9. I/We understand that the preschool does not hire outside professional consultants. The state’s licensing agency (Department of Social Services: Community Care Licensing Division) has the right to inspect the facility and files and interview personnel and students at any time, including unannounced visits. Parents have the right to enter thepreschool at any time, as outlined in the Parent’s Rights Form, while their child is in attendance.*

10. I/We agree to honor the preschool’s Catholic education philosophy and mission. I/We pledge to support the administration and the faculty of Holy Cross Preschool and will keep the lines of communication open by following the “Guidelines for Communication” in the Holy Cross Preschool Family Handbook. Acceptance of students into a Catholic preschool means that the family of the student is also accepted into the school community.*
