申請者須知 Notes to Applicants：
1.) 12 歲以下小童須由家長或監護人陪同參加本會活動。
Participants aged below 12 must be accompanied by a parent or guardian.
2.) 家庭會員之會籍只包括申請人、配偶及兩名 18 歲以下子女。
Family Membership includes the applicant, his/her spouse and two children under 18 only.
3.) 學生會籍適用於12歲或以上之全日制學生。申請時請附上有效的學生證副本（需印有
申請人姓名及相片）。
Student membership applies to full time student aged 12 or above. Applicant should submit the copy of a valid student card when applying. (The student card must include applicant’s name and photo.)
4.) 會籍一經本會確認即時生效，至2020年12月31日。成功申請者將收到
電郵/Whatsapp確認，如提交會費兩星期後，仍未收到任何通知，請電郵或致電與本會
職員聯絡。
Membership will be in effect upon confirmation from ISHK office, and is valid for the year of application. Applicants shall receive a confirmation via email or Whatsapp upon membership approval. If the applicant does not receive any notification, please contact us by email or phone.
5.) 會籍及會員卡只供會員本人使用，不可轉讓。
Membership and membership cards are not transferable and may only be used by the named Member.
6.) 申請人、家長或監護人在本表格簽署後，即同意豁免本會於各項活動中, 如發生意
外或不幸事故時對有關人士責任。
By signing this form, applicant or the parents / guardian of a child under 18 absolves any responsibility in the case of accident or mishap in outings & all activities organised by Ichthyological Society of Hong Kong.
7.) 香港魚類學會保留所有有關此申請的最終決定權。
Ichthyological Society of Hong Kong reserves all rights regarding this Scheme.
個人資料收集聲明 Personal Information Collection Statement
申請人提交的個人資料將絕對保密，有關資料只會用作與香港魚類學會有關活動之用。申請人如需要查閱或修改所提供的個人資料，請以電郵方式向本會職員查詢。
The personal information provided will be kept in confidence and provided to relevant staff for application processing, contact, and relevant activities organised by Ichthyological Society of Hong Kong. For enquiries or amendment to the provided information, please send us an email request.
