Degree Change Request Form

If you are completing the Undergraduate program at Grace and would like to enter the Graduate program at Grace you will need to complete an application (not this form).

If you are completing the Graduate program at Grace and would like to enter the Doctorate program at Grace you will need to complete an application (not this form).

If you wish to change your degree from one Academic Catalog to the most current Academic Catalog, please complete the General Petition (not this form).

If you wish to change from one degree to another degree in the same program (Undergraduate or Graduate), please complete this form.

If you are requesting to add a degree path, you will need to work towards completing the lowest degree path first (as the course schedules allows). Please be advised that if you complete more than one degree path at the same time, only the highest degree path will be recognized at the graduation ceremony. However, the graduation fee is to be paid for each degree (diploma) received. Please contact the Registrar (registrar@gsot.edu) with any questions regarding this.
