General Terms
Cedar Creek Pet Hospital will exercise reasonable care for the safety of your pet, and to keep the boarding premises sanitary and properly enclosed. Animals will be fed properly and regularly, housed in clean and safe quaters. Cedar Creek Pet Hospital cannot guarantee against accidents, and absent negligence on our part, we cannot be liable for loss or damage by or to the animals at this facility. Owner agrees to be solely responsible for any and all attacks or damage caused by the owners pet while it is in the care of the Cedar Creek Pet Hospital.

. Sunday Pick Ups: Cedar Creek requires payment in advance for Sunday pick ups so please allow time at drop off. The pick up time is from 5 to 6 pm. Pets must be picked up by 6 pm, if not picked up by 6 pm an additional Night Stay will be applied.

. Monday-Saturday: Pets must be discharged by 12 pm or an additional half day of boarding will apply.

Vaccines
In order to protect the health of your pet, this facility requires documentation showing that all boarding dogs have current Rabies, DA2PP and Bordetella vaccines, and cats have current Rabies and FVRCP vaccines. If any of your pets' vaccinations are past due, they must be inoculated before entering boarding facility. Vaccines that must be administered at this facility will be added to your bill. Pets that are so young that they have not completed their entire series of vaccinations may not yet be protected and, thus, owners accept any risk of infection.

If your pet will be receiving medication during his or her stay, it must be in the original veterinary-labeled container with instructions for administraion and your veterinarian's phone number.

Geriatric Pets
Older pets may experience additional stress in the boarding, daycare, or grooming environment. Cedar Creek Pet Hospital is dedicated to providing exceptional care for pets, including geriatric animals. Your signature acknowledges that you are aware of any age related risks to your pet. We do not offer a "Do Not Resuscitate" option for any pet staying at Cedar Creek Pet Hospital regardless of age.

Personal Items
Cedar Creek Pet Hospital will not be held responsible for any lost or damaged personal items.

Abandonment
If your pet(s) are not called for within 14 days after designated time, the animal(s) may be placed for adoption at Cedar Creek Pet Hospital's discretion with or without an adoption fee. Advance notice in writing of any such adoption will be sent by registered mail to the address of the owner given hereon, and no further notice shall be deemed necessary. All adoption fees will be the responsibility of the owner.

Payment/Nonpayment
Cedar Creek Pet Hospital charges for boarding accommodations by the day. Owners agree to pay the rate for boarding in effect on the date the pet is checked into the boarding facility. Boarding rates are assessed based on the pets weight at the time of check in. If the charges are not paid within ten (10) days after invoices. Interest will accrue at 8% per year. Any collection expenses, including attorney fees, shall be paid by the owner. Owner further agrees that the pet shall not be permitted to leave the pet care facility untill all charges are paid by the owner. Checkout time is 12:00.

This Authorization and Release will remain in force for all my pet's visits to Cedar Creek Pet Hospital, for any services including but not limited to boarding, grooming, or daycare.

I hereby agree to the foregoing as the owner of the aforementioned pet(s). I further certify that my pet(s) is (are) in good health, have not bitten anyone in the past 10 days, have not been ill with any communicable condition nor exposed to any communicable diseases within the last 30 days. Moreover, I certify to the accuracy of all information given about my pet(s) and that my pet(s) have not harmed or shown aggressive or threatening behavior toward any person or animal. I have read and understand the foregoing.
