1. There is possibility that the prescription drugs received from these vendors may be counterfeit because they have not been approved by the FDA; and the pesticides may not have been approved by the EPA. This has occurred in the past and could recur.
2.Due to the fact that the FDA does not regulate any of the products on-line by making sure they are stored properly, not expired, or illegally imported from other countries, there is no guarantee that the product will be effective.
3.You may not have the medication available soon enough to start your pet’s treatment in an optimal time to provide relief for his/her condition.
4.When prescription medications are purchased elsewhere our staff is not able to go over potential side effects with you or show you how to dispense the medication.
5.Manufacturer rebates that would ordinarily be available for products purchased from this facility are generally not available.
6.The manufacturer’s warranties or guarantees for these products may not be valid. This means if your pet’s condition is not effectively treated with the product(s), manufacturers may not stand behind their products or product liability procedures. Additionally, the owners of and doctors at this facility will be unable to assist you in claims against those manufacturers.
I have read and understand the above, accept these risks, and am aware that I assume all responsibility associated with ordering and administering these medications to my pet, releasing Loving Family Animal Hospital, and its entire staff from any liability that may arise from complications related to receiving these product(s).
