    I — Medical and/or Surgical Authorization

    I hereby authorize and direct the Avon Lake Animal Clinic to hospitalize this animal for the purposes of diagnosis, treatment, surgery, or other procedure(s) as specified previously and approve the use of whatever anesthetic deemed advisable for the well-being of the animal. The nature of such services has been described to me to my satisfaction. I realize that no guarantee nor warranty can be ethically or professionally be made regarding the results or cure. In emergency circumstances I also authorize the Avon Lake Animal Clinic to perform such procedures as they feel are necessary for my pet's health and well-being. I realize that my pet will be discharged only during regular clinic hours and when Doctors or their associates are present and further, I assume responsibility for all charges incurred to the patient.

    II — Laboratory Tests Waiver

    If your pet is to have an anesthetic, rest assured that advances in anesthesia and surgery have made routine procedures relatively safe, with a low rate of complications. Nevertheless, occasional problems can arise due to pre-existing conditions not evident during routine pre-anesthetic and pre-surgical examinations.
    To better evaluate the ability of your pet to undergo an anesthetic we strongly recommend certain laboratory tests based on their age and general health. These tests will be performed (and you will be billed for them) unless you refuse them by signing below.
    I have been advised of the preanesthetic screening laboratory procedures recommended for my pet. I decline these procedures and hereby release the Avon Lake Animal Clinic from any liability that may arise from use of anesthesia for my pet.

    Ill — Intravenous Catheter and Fluids Waiver

    During an anesthetic procedure, anesthetic agents are removed from the body by lung, liver and kidney. Intravenous fluid therapy helps maintain blood pressure, vascular flow thru liver and kidney and provides an existing intravenous port in the event of emergencies. The American Animal Hospital Association recommends that all pets undergoing general anesthesia are maintained on intravenous fluids. I have been advised of the IV fluids recommendation for my pet. I decline these procedures and hereby release the Avon Lake Animal Clinic from any liability that may arise from the lack of use of IV fluids for my pet.

    IV — Boarding/Grooming Release

    I hereby request, consent and authorize the Avon Lake Animal Clinic to board, care for and treat the above described animal(s). All animals entering the clinic should be up to date on vaccinations and free of external parasites (fleas, ticks, etc.). Should an illness or emergency arise, I also authorize the Avon Lake Animal Clinic to perform such procedures as they feel necessary for the well-being of my pet(s). Further, I assume financial responsibility for all charges incurred to the pet(s).

    V — Tranquilizer/Anesthetic with Grooming/Bathing Release

    If a tranquilizer or anesthetic is necessary for the humane handling, grooming or bathing of my pet, I give my permission to the Avon Lake Animal Clinic to administer such medication.
