1. I understand that my participation is voluntary and that I am free to withdraw at any time before July 2019 without giving any reason.

2. I understand that data collected during this study will be processed in accordance with data protection law as explained in the Participant Information Sheet.

3. The questionnaire data will be stored on a password protected computer and accessed only by the researcher, this data will be kept for up to ten years and will be disposed of when no longer needed.

4. I understand that the results of this study may be published and / or presented to the Senior Leadership Team at the school. I give my permission for my anonymous data, which does not identify me, to be disseminated in this way.
