AUTHORIZATION: I hereby authorize The Veterinary Wellness Center of Green, its clinicians and their assistants to treat my pet in the manner that is considered to be necessary based on their clinical and/or diagnostic findings. I assume responsibility for all charges incurred in the care of the animal. I also understand that professional fees are due at the time services are rendered.
