    Every attempt is made to know the temperament of every pet before pickup is made. While it is nice to know that a dog is happy or nice, we are looking for any known behavior that may manifest as dangerous to the pet or handler.

    Please note that some pets will behave different when placed in a stressful situation. Behaviors such as chewing, snapping, scratching, biting, eating things off the ground are what we need to know to ensure we can make the necessary provisions. Dogs known to be chewers will have limited use of blankets in their kennels, as they may ingest them.

    Gill Bright Animal Hospital agrees to provide all pet care services in a reliable, caring, and trustworthy manner. I understand that it is Gill Bright Animal Hospital’s (GBAH) sole responsibility, with regard to my pet(s), to act with reasonable care. If my pet becomes ill, injured, dies and/or escapes while in the custody and care of GBAH (whether such illness, injury, or death is discovered while in the custody of GBAH or afterwards), I agree that if GBAH acted with reasonable care, I shall not bring any claim, suit, or action of any kind against GBAH arising out of the illness, injury or death of my pet(s).

    I understand that there are inherent dangers in the transport of animals that is beyond the control of Gill Bright Animal Hospital, and therefore, I release GBAH from any liability in the event of my pet’s injury or death during transport.

    Cat bites, in particular, can be dangerous and require medical attention. Due to the serious nature of the bites, drivers do not place cats in their kennels or chase them upon arrival. It is important that all pets on board trust the driver and it is our belief that it is best for the pet parent or pickup contact be the one to kennel the cat(s). If the cats are not in a carrier or isolated to a small room where the owner can easily gather them, it puts everyone at a disadvantage and the driver may leave the pickup location without boarding the cats. A no show fee will be charged to the card on file. The drivers are instructed not to place the cat(s) into the carrier or kennel but it is expected the owner or the pick-up contact will place the cat(s) in their carrier(s) or kennel.

    In the event of an emergency, GBAH will make every attempt to contact the client. If emergency veterinarian care is needed, I authorize Gill Bright Animal Hospital to obtain those services. GBAH will not be responsible for additional charges for emergency care unless care was needed due to the negligence of Gill Bright Animal Hospital.

    On most transports, space is limited. We can only accept a tote or square container that is approximately no larger than a five-pound bag of pet food. In addition, you may send a pet bed. Anything larger than this must be cleared in advance and will be based on the availability of space on board at the time of transporting. We reserve the right to limit the amount of bedding, toys, food and medicine we take with us.

    I understand that failure on my part to meet the terms of this agreement or the Gill Bright Animal Hospital policies and procedures will result in GBAH being unable to transport my pet. I understand that GBAH reserves the right to stop service to me and my pet(s) at any time and for any reason.
