Please provide a copy of your super fund bank statements/transaction history for the period 1st July to 30 June.
If this can be obtained in csv format please provide that to us.
We need this for all accounts, including term deposits and share trading accounts, as they are required for audit.
In order to accurately process the bank transactions, please ensure there is enough description next to each transaction.
Please forward a copy of the ‘ETP Roll-over Statement’ provided by your previous super fund for any benefits rolled into your self-managed super fund during the period 1 July to 30 June.
Share / Managed Fund Purchases and Sales
Please provide details of all shares and managed funds purchased and sold during the period 1st July to 30 June. The following documentation is required:
Transaction/Contract Note History’ from your broker for the period 1 July to 30 June (Please include buys/sells in a single report and sort in date order)
Managed fund purchase confirmations
Copies of any Off Market Transfer forms
Details of any dividend reinvestments, share buy backs, bonus shares, rights issues, IPO’s, share purchase plans, mergers, share consolidations etc
Other Asset Purchases and Sales
Asset purchases and sales
Please provide details of other assets purchased and sold during the period 1 July to 30 June.
Documentation evidencing the purchase and sale must also be provided showing the owner as the super fund.
This could include documentation such as contracts, settlement statements and statements of adjustments.
Income from Assets
Please provide details of all income received from your funds’ assets. Documentation required to be forwarded includes:
Share dividend advice
Managed fund distribution & taxation statements
Details of any other income
Please provide details of rental income and expenses for the year including annual agentstatements, copies of rental deductions, depreciation reports, loan statements (if LRBA).
Please provide a copy of the annual insurance statement or policy schedule for each member insured.
This should include the owner of the policy, premium amount paid for the year, type of insurance cover, if it includes TPD then whether it is any or own occupation.
Summary of Shares / Managed Funds owned at 30 June
Share and/or Managed Fund Summary
Please provide a summary of shares/managed funds owned at 30 June. Please include number of shares/units held. If you use a broker, please include the year-end summary. If no broker used please provide statements from fund manager, companyor share registry showing units/shares held at 30 June.
Market Value of Assets owned at 30 June
Please provide a summary of assets owned at 30 June and the market value of the assets at 30 June. It is now a legal requirement that all assets are valued each year at market value on 30 June.
Please provide a valuation or other documentation to confirm the market values.
Please provide evidence of the storage for your collectible including any invoices issued and paid.
Please also provide copy of insurance policy in super fund name for the collectable and the evidence of market value at 30 June.
For existing pensions – copies of PAYG summaries (if applicable). For new pensions – copies of resolutions, pension schedules and compliance documents.Please notify us if you would like us to prepare any required documentation for you.
Lump Sum payments
Lump Sums - Details of lump sum payments, resolutions and ETP paperwork if applicable. Please notify us if you would like us to prepare any required documentation for you.
Rollovers - please provide details and documentaion of any rollovers to other super funds including resolutions and ETP paperwork. Please notify us if you would like us to prepare any required documentation.
Details of all expenses for the fund for the year including tax, audit, accounting, investment expenses etc. If these were not paid by the fund, please provide details of how they were paid and if they are to be reimbursed to the payer. For expenses that are not directly fund expenses, you may need to complete a Trustee Declaration confirming the expense is 100% for the fund. Please contact our office for this declaration.

Copies of resolutions for the year.
Copy of updated investment strategy (if prepared yourself).
Record of all members at 30 June.
Employer Contributions - Please identify the deposits that are employer contributions, and which member they relate to.
Non Concessional contributions
Non-concessional contributions - Please identify which deposits are non-concessional contributions and which member they are for.
Personal concessional contributions
Personal Concessional Contributions - Please identify the deposits that are personal concessional contributions and which member they are for.If a member is claiming a tax deduction in their individual taxation return then the member must provide a notice of intent to claim declaration to the super fund, and the fund must provide acknowledgement of this.Please provide these documents to our office, or if you wish for us to prepare these please let us know.
Spouse Contribution - please identify the deposits that are spouse contributions, and which member they relate to.
Governement Co-contribution - Please provide the paperwork from the ATO identifying which member/s and amounts the co-contributions are for.
Small Business Concessions
Small Business CGT concessions - please provide ATO CGT cap election form and any advice received or other documentation regarding the application of the concessions.
