Family Security Card Application Form
Family Security cards are issued to all Stamford families. Up to three Family Security cards will be issued at no charge per family. This card is used to gain entrance to the school for child pick-up and for parents to school events. Please be sure to fill out below and upload a photo(s) as we will not allow temporary parent passes to be used after September. Please use only one form per family member. The names and contacts provided in this form will also be used as your child's emergency contacts.
